Thank you so much for purchasing the CEU course: Book Club – Legacy by Uché Blackstock, MD! Below are all the links to the quiz and evaluation. Both need to be completed to get CEU credit. Payment must be received before a request for CEUs will be submitted.
1) The first things you will need to do is borrow or purchase “Book Club – Legacy by Uché Blackstock, MD.” If you purchased the program “with book” then lookout for an email from Brittany in the next few days with your shipping information.
2) A PDF of the final quiz is located below. You may want to print a copy and complete the quiz as you read. The questions are broken down by chapter. This step is not required!
3) You will need to submit the following electronic quiz (same questions as above) and get an 80% or higher, within 3 attempts. When you finish the quiz you will see your score in the top right as a fraction and you will need to do the math to make sure you got an 80%. If not, you can take it again (up to 3 times total). The email saying you passed the course is manual, and will arrive within a couple business days. So when you don’t get a confirmation of completion right away, don’t worry:

5) You did it! Your official CEU certificates will be issued by the end of next month through your NSGC portal. We are no longer offering expedited certificates.
* Of note, 12/1/26 is the last day to complete the course materials for The CEU Shop programs to ensure your CEU certificate will be issued in your NSGC portal in 2026.
